Called and said someone was making charges to some account, which I didn't know what it was and said push 1 to get a tax form.  I'd like to know if this is some hoax.

I also received a message on my  machine from this number, making the same claims as above. Is it legit or is it some fraud.

I recieved a call from this number several minutes ago.  Mentoined a PRN number also?????  when i called back i recieved a fax tone.

I just got a call from this, same as others have described.  They should be required to identify themselves better.

I received a message from this phone number, but the message was truncated as though it was missing the first part of the message.  I am wondering if it is somehow related to health insurance as I am a health care provider.  Are any of you health care providers?

I recieved an incomlete messg, from the same #,, I am going to report this # to the FBI ,, I am sure it is a king of scam and it is illegal

I just received a phone call from this number. They said it from the IRS and I needed to push 1 to receive a form via fax.

We received a call from this number today with similar recording about IRS...push 1.

Same thing as above.  I returned the call, only to get a fax tone.

I received similar calls. I have also had problems from another number 469-521-0539 belonging to a company called NRS. Don't be fooled. These are only scammers calling for personal information, or scare you into sending money for a debt you don't owe.
I do not know who to report these calls to, does anyone out there have any ideas for reporting this fraud.

I just received this call at 3:14 pm EST on November 13th.

I thought it was strange so I googled the number and found this page.

I own a small business and at first thought it might be a tax form related to my business, but my accountant would have mentioned something like that.

Because the caller did not identify themselves I don't think it is legitimate.  I'm glad to see there is a service like this website.

i received this call too with a message about a tax form. seems like a total scam. dont be fooled & dont give any info. i just hope they stop calling.

The exact message was: "Insurance carries and payers accessing PRPN [or TRPN] have reported members using your services and making payments to you in 2007.  In order to receive copy of an IRS W9 along with PRPN [TRPN] provider participation agreement press 1 if already received a copy please review."

Something about a tax form and someone using my account.  Called back and got a tone of some kind.

Please go to donotcall.gov to report this #619-677-2480 phone calls.  File a complaint or it will never stop.

Received a call whle I was out a portion of which was recorded on my answering machine.  It stated "... insurance carriers and payers accessing PRPN have reported members using your services and making payments to you in 2007.  In order to receive a copy of IRS Form W-9 and the PRPN providers participation agreement, press 1.  If you have already received it, please review immediately."  

When I attempted to call the calling number as recorded on our caller id back, it answered as if it was a fax machine.

Got another fax wanting private information from "3 Rivers provider network" from this extension.  Second time.  This time, I filed complaint with Federal Trade Commission and FCC.  F.U. 3 rivers!
