They seem to be a collection company called UCB.  Called this number but they did not answer.

it is from indiana dept of revenue for past due taxes/ address is p o box 1120 maumee oh 43537  better check taxes owed  trust me

This company United Collections Bureau works for the State of Indiana Revenue Dept and will do whatever it takes to ruin you.  I am in the works. w the State and they failed to notify UCB.  UCB levied my bank accts.  State of Indiana told them to release the levy due to an error and UCB said, "NO".  So I guess even though they work for the State it doesnt matter.  This issue has turned my credit upside down in a day.  I have worked w. the State calling, faxing my information over to make sure that nothing like this happens.  It was great that the State was quick to notify UCB and tell them to remove levy but if the collection agency has that much power what difference does it make.  

I called the collections agency and spoke w. Mr. Lyons and he keep telling me that I failed to pay when I filled.  I knew about since April 2008.  When all along this is due to an overpayment and not a failure to pay.  Rec'd notification from the Revenue Dept. back in May of 2011.

They have called a wrong number and sent one letter.  So if they call immediately call the State of Indiana. I am now emailing the Governor and will contact local media to let this company know what they are doing is an injustice.  I am not the first and will not be the last.

A hispanic male called me stating he worked for the state revenue board and needed my SS #. When i refused he hung up. This is a fraud .
