Call on my cell phone stating they were conducting a survey about the swine flu H1N1 virus and to please call them toll free on 800 993 0495.  The number on caller ID is 312 759 4899

Same thing happened on my WORK PHONE.  312 area code on my caller ID.  Left a voice mail for a call back regarding flu survey.  They have called every day for last 2 weeks.  Something is not right.

They called me about 6:30 P.M. Seattle time and said they were conducting a survey about the flu. She was hard to understand as she did not seem to be a native english speaker. She said she was from the Center for Disease Control. But she didn't say Atlanta. I said please don't call me anymore.
The next day there was a message very much the same saying to call her at 1-800-993-0495. Seemed a little fishy to me so I Googled the phone number and wound up here.

Upon further research of this caller, they are legitimate-you can check out NORC at the University of Chicago...

Professional Privacy Invaders who deserve nothing less than a loud whistle or airhorn in their ears. Actually they deserve far worse but technological limitations of the phone system prevent giving it to them.

You have no idea where the information you might give them goes, what it is used for, or how it is protected. PROTECT YOURSELF. Do not cooperate with these privacy invaders.

I got the same message.  They said they were from the "Center of Disease Prevention".  Most people that work there (friends included) call it the "Center for Disease Control".  Didn't call back, but definitely fishy...
