Yes, some stupid medical survey, they call every single day and even left a voicemail.  They say they're from the city of Chicago.

Yes, they called my mom about a medical survey just now, not sure whether it's legit or not, but they told my mom (who answered the 35-minute survey!) that they'll send her a $10 gift card for answering. Ugh, I don't know why she agreed to disclose her address, but it's done.
Anyone else answered them (and received the "gift card"?)

Update: If you try calling the number back a pre-recorded message always says that "the person at extension 103 is unavailable, please leave a message after the tone". Definitely a scam!

Call all hours of the day, just called now on a Sunday @ 515pm and never leave a voice mail. Why would the city of Chicago call on off hours?!?!?

No name.  Why is City of Chicago surveying people several states away?  SCAM!

Called me 4:42pm - left no message.
