caller left voicemail that included offer of gift card.  called United Healthcare, gave them the number (which may be theirs actually) and let them know it sounded like a scam.  it may be a valid offer;  but why ?

Yea I guess it is United health care but sure sound like a scam trying to send medical to my house.  Didn’t want to hang up even when we said we visit our doctor in his ofc.  I am blocking this number

Part of their "Home Healthcare Check". It's an annual member "benefit" for the insurance carrier. They are trying to be proactive and preventative for members, but it is a contractor who sends nurse practitioners, or ?? to run the checkups.
When you participate, there is a gift card with $$ or an alternate gift. This way they hope to catch potential medical or health problems early.
I've done it twice, after refusing for a few years. It's an inconvenience, and time consuming, but if you don't visit a primary or specialist regularly, it's good to make use of the medical review.
