CANNOT VERIFY ANYTHING ABOUT THIS COMPANY. PEOPLE SENDING DATE OF BIRTH, SOCIAL SECURITY NUMBER, CHECKING ACCOUNT INFORMATION, AND SIGNATURE.  COMPLETE INFORMATION FOR IDENTITY THEFT IF THE COMPANY IS A HOAX.

Address seems to be New Health Benefits; 3838 Ramert Dr, Ste 3; Las Vegas, NV 89121 - company only in existance from July, 2007 according to NV Sect. State records. Two phone numbers: initial fax: 800-715-3712; Follow up no: 800-516-7038. One of hundreds registered by XTREME BUSINESS SOLUTIONS, INC of same address, many of which are Default or Revoked registrations.
Customer Service refers to a NJ company: Professional Benefits Consulting, Inc. 856-374-8665 in Blackwood, NJ - cannot find info on them.

This is a fax line that the Group Health care gives you to fax the info.
Very fishy...when you speak with the rep. at 800-615-1174 ext 411, she cannot tell you the name of the company she is submitting the insurance application.  When I asked her for the physical address of Group Health Care, she gave me a Las Vegas, NV address, then her supervisor got on the line and I asked him about the Phoenix, Az. address and he said he could not divulge it because there had been a rape in the parking lot????? When I asked him what the parent company was he said National Alliance, nothing found under that company either...Supposedly the company is New Health Benefits, a Las Vegas, NV corporation, again,  I asked the supervisor who was Xtreme Business Solutions, Inc. he said he didn't know; in the Nevada Sec. of State site it lists that company as the registered agent...Too many discrepancies...There is no listing for Group Health Care in Phoenix, AZ. I could not find the 800-615-1174 under any listing, since I don't have the real name of the company....

I had this same thing happen to me! Only I was stupid enough to give them my information!!!!!!

This company is a HOAX. No company name, they do not accept credit cards and ask for your checking account information!!!!!!
I was told that they were part of Beechstreet but Beechstreet do not know anything about them.
