same for me, he said he was with military dental and set up a dentist appointment for me.  I use a different dentist and he advised me of a form to have filled out, which the form is official - DD2813.

i too was called by this number they said to be checking up on my post deployment health reassesment usind military form dd 2900. they did sound legitimate by i have some doubts. all question asked were question ive answered on the form.

i too just got a call from them, they stated they were with the US ARMY RESERVE READINESS PROGRAM about medical stuff... She knew my rank and everything... sounds legit enough to me, but they referred me to another 1800 # to call back at..

I got a call from this number and they said they were from the post deployment health reassessment and gave me another number to call back.  I called them and talked to this guy who knew my military information.  He asked me for my SSN# and I gave it to him and now I think I am in trouble!

This company is legit. I work with them. The name of the company is Logistics Health Incorporated. They are in LaCrosse WI and they have the DOD contract for all of the military readiness medical programs including Post Deployment Health Reassessments, Periodic Health Asssessments and Immunizations.

I would trust them but they called it a post deployment health assesment and got my rank wrong, I was promoted 2.5 years ago and redeployed to conus 3.5 years ago

Got a call from this number for medical readiness dental exam.  As I've gotten these calls just about yearly, and they all sound the same I'd guess it's legit.

Has anyone actually set up an exam through them yet? I have my own doctor and dentist now I don't feel I need to be assesed by the military anymore. I go to a real Doctor and Dentist and my dentist is having to re-work a lot of the stuff that the military dentists did. Don't plan on going to another one. Sorry

This Call Is for the Army Reserve Dental program and it is legit I've set appointments with them before. it's normaly wiwth a civilan dentist and the army picks up the tab....

I didn't answer this call to my cell phone because I didn't recognize the number.  They didn't leave a message, so I Googled the number and found this site.  I don't know if it's the same person talked about here since I didn't answer it, but thought I'd contribute in case this is still going on.

I receive a call from them each year to schedule exams. They are legit.
You can call their 800 # at 1-800-666-2833 M-F 8am - 11pm eastern time or Saturday 8am - 4pm eastern time.

If you are getting a call from this # it means that you are in the USAR and it is time for your annual PHA- periodic Health Assessment. Do NOT ignore it. Yes it is "legit". Your unit S-1 or Admin NCO should have instructed you to fill out your PHA on line and then call the 800 to schedule your appt. This is an annual requirement and has been for some time. Get with the program folks. Its part of being in the Operational Reserves. ccc-sorry your dental work was shoddy, but pre-deployment dental work is a quick-fix and provisional-its not meant to be pretty or permanent. If your teeth are jacked up, get 'em fixed before you deploy. See a dentist annually, and get your teeth cleaned every six months. Same goes for preventative medical care. Take care of yourself. Don't expect the Army to fix all your ailments once you're deployed.

This number is the Readiness health Assessment place. It's legit if you are in some branch of the military.

This is the logistic health number from lacrosse wisconsin. They set up medical appointments for companys and the armed services. They set up appointments for Uncle Pete as well.
