This outgoing number has shown up for the last 5+ weeks on my home phone's placed call list. I did not place any calls to this number. Call duration goes from 0 to 90 sec. Some how my phone number has been hacked. Number has been placed on a call screening list to stop the calls from being made

I have got details on my outgoing call log of calling this number hundreds of times between midnight and 4 a.m.
I have never called this number! How are they listed on my phone as outgoing calls? I live alone and there is nobody else that was here to make the outgoing calls.  They need to be stopped and investigated as to how and why they are hijacking phones

If your call logs show your account has made outgoing calls, then thats because it has. If you do not allow others to use your phone, check with your service provider and have them check for other devices that might be on your account without your permission, such as a cloned phone.
Caller ID spoofing is simply masking the outgoing caller ID with a number other than the real one, but has no direct connection to the spoofed phone number's account and won't be logged, since its never connected to your service.

Multitude of calls appearing on my OUTGOING call list.  I have NEVER called this number.  There is no information about it but I want it to stop immediately.  Where can I go to get this off my phone and out of my life?

Shows outgoing calls made to this number a multitude of times every single day.  I have never called this number.  This is my private fax number.
