Received a letter in the mail addressed to "Maine Resident" wanting me to take "an important survey to learn your thoughts about health care in this country. The survey is being conducted by SSRS, an independent survey research firm."
"Your generous participation is critical to the success of this project. If you qualify for and complete the survey, we will send you a $10 check in the mail or a $10 Amazon gift code as a thank you for your participation."
Needless to say, the survey was not taken by phone or internet.

Received another letter stating "Recently, we sent you a letter and postcard inviting you to participate in the Health in America Survey. If you have already completed the survey, thank you? If you have not yet completed it, we hope you will take a few minutes to help us."
"SSRS, an independent survey research firm, is conducting this study to learn more about your thoughts, especially in regard to health care in the U.S. The results will be used to help policy makers better understand what people across the country think and how they fell about their health care."

Just trash them. Anything mentioning a 'gift card' for participating just sets off red flags.
