They seem to know the person information such as, name and address. Sound real at first. First call ask for my address... then they end the call. I call back to the number and different person pick up I play along with what they said.

Most recent calls start from 310-695-1808/x-x-1834, 630-364-3184, and 760-375-3572. This 855# is their main line.  New DBA name is United Pharmacy Services, same address in FL for Custom Services LLC and CV McDowell LLC dba Express Rx Pharmacy Services..

Called me  this morning from the same number but they have changed their company name from United Pharmacy to First Care Services.  Still claiming that I had filled out a survey for chronic pain--which I do not have--and wanting my insurance info.  Complete Scam.

I too received a call from First Care Services and fell for it, I called FTC and FBI

Say they are first care service,  saying they can offer a pain free cream,   then they want to verify information,  and of course then they will get Doctors approval.  I told them no need to verify before I answer any questions with insurance.  I ask them where were they getting this information and they said something like it is on your wellness form.  I'm sorry but didn't sound right.  Did not give them information. I asked for a number so I can verify they have me 8553713933.

See odd.  Hung up and decided to call back, same spill different person, but she knew my name and started  the spill before I said a word. I asked her where was she getting this information, she said a questionnaire I filled out.  Did not give them information asked her to remove my name and number. She well most people don't have a problem with us. Way to many flags.

They called asking for my 16 year old daughter. When I identified her as a minor and asked where they got her information, the caller quickly apologized and promised to delete her information. Not sure if they will, but they weren't willing to scam a kid.
