    Has also sent 3 mailings.
    Person that was on Medicaid has passed away 5 years ago.
    Promises $20 to take survey. BUT NEEDS BANK ACCOUNT INFO OR CC NUMBERS INCLUDING SECURITY CODE ON BACK to PAY YOU !!!
