Got a call, told me to reference a case #. Looks like they are a clearinghouse for medical records for my life insurance application.

Examination Management Services, Inc. They are hired by the life insurance company I applied with to investigate my medical records. They provided a case number and asked for a fax number or email address to which to send a release form.

This number called the office a moment ago. If this is a medical/insurance company, it was odd that they asked to speak to someone who is deceased. I was so taken by the request, I hung up out of instinct.
