No one here can do as you request. This is simply a site to help identify unknown phone numbers, not a fax-stopping service.

Received a fax declaring "NEW Reduced 2017 Life Insurance Rates" from a company claiming they "are licensed and certified life insurance specialist- offering many plans" but nowhere on the document do they actually identify themselves. They don't even give a phone number for contact, just say to fax this number for a quote.

I checked them out - they are a Canadian company with the same 888 number that works only in Canada.  It is a duplicate to the US 888 number that works only in the US.  Not the same - unfortunately.

Response number given on junk fax sent from 972-598-0255. I have blocked both numbers.

Fax received last night 4/4/2018 at 3:22AM !!!
Title of fax: Affordable Life Insurance for Medical Professionals.
Faxed Paper shows rates for insurance for male and females of different ages and dollar amounts. Includes the fax number. No other info as to who sent it. Asked for my info to be faxed back to this number to receive a quote..

Yes, nothing like putting your phone number up on the internet to stop calls.  You can expect a lot more calls now.  Post reported to try to mitigate the damage you did to yourself.
