I received the exact same information as this with the exact number and company stated as Data Serve Team.

Luckily I wasn't victimized as it all sounded very suspicious. Also, I was directed to go to this site AFTER I submitted the information (which I didn't submit the original) and that site was http://data-serve.net/apply.php.
