I didn't get a call from them, but they just charged my credit card for $9.85 without any authorization from me.  I did get a weird call a few days ago (not sure if same number) from someone that said they were just calling to confirm my phone # then hung up.  Sounds like a scam going on.  I've disputed the charge with credit card company.

Just like Matt, I didn't receive a phone call but I am showing a bogus charge on my credit card for the same amount of $9.85. It showed up on my account has "form limited from Coral Gables, Fl". My bank is showing it has an online dvd company. It definitely sounds like a scam.

I also got an out-of-nowhere charge on my credit card, though mine was just $9.80, labeled "Visa Form Limited 888-512-1796 FL".

I Googled the name. I feel bad for a small home design company called Form Limited in Connecticut; I bet they're going to get a lot of heat for this scam even though I doubt if they're at all involved.

In any case, I'm calling my bank to complain. I'm glad to know I'm not the only one.

Quick follow-up.  My credit card company called me about the disputed charge.  They said it was for a DVD rental.  I asked if they had any paperwork showing I authorized the charge.  Of course they did not.  I told I looked up the phone # online and it was for a collection agency.  They agreed to reverse the charge.

Did they state who the DVD Rental was from?  If this is the case then perhaps it is the same for me although my bank stated that they had no idea who it was and that if I wanted to do anything I first had to cancel my credit card and then they would look into it.  I have tried to call the number but get nothing but a recording and it sounds like an Australian male telling me that my call has been forwarded because there is no one to answer.  I have tried calling all day; please let me know who you called to find out it was a DVD rental company.

I didn't get a call. I got something even worse. Looking at my bank statement, I got a charge on my debit card, and it listed 1-888-512-1796. I called it and got a British accented voicemail that clicked me away until it disconnected me. I had to cancel my debit card with the bank, and I'm now fighting the charges. The bank asked me if I were willing to press charges .. absolutely. It was "only" $19.95, but bank fraud is bank fraud. Well, wish me luck.

I did not receive a phone call from this number but my bank did catch the $9.85 credit card charge for a "video rental" which these people tried to sneak through.  Luckily the bank called me regarding the charge, I did some research, and found the fraud through this website.  I don't understand why these people can't be located and shut down.  They are undoubtedly making millions off of innocent people who don't catch the fraud.

Called the number and got an Accented (Australian?) Recording: "Your call has been forwarded to an automatic voice messaging system. Sorry, but no one can take your call right now. Please leave your message after the beep."
Cancelled my card and will dispute.

Has anyone received recurring charges?

They did not call me but had a charge on my debit card for $9.80 in Feb. Then on 4/7/08 had a credit for $9.80 on my debit card from form limited. Has any one else got a credit back.

I have unauthorized charges ($9.80) placed on my bank statement, from someone called FORM LIMITED @ 888-512-1796. Does anyone know anything about this company?

I called and found out it was for a video of some kind.  canceled my card
number and requested a new one.

I noticed the same charge on my bill "Form Limited 888-512-1796 Fl" for $9.85 dated 4/02/08.  I called Visa and they cancelled card and will send a new one.

Same with me...charged 9.85 on 4/28/08...bank is investigating.  form limited.  I live in florida...are any of you in florida too?

Got a charge on my debit card for $9.75 on 3/31/08.  I was blaiming my husband for it then realized it was my card.  Thought nothing of it until I Googled the phone # the bank had for the charge and saw all your comments.  Had the bank cancel my card and investigate.  Thanks everyone for your tips!!!
