Am getting a LOT of calls from this # !   If anyone can help me with some information on who this is,

I keep getting calls from this number and I would appreciate it if you would STOP calling. Take me OFF your call list immediately

I did some research on this number as it was on an official looking letter from an address in Washington DC regarding "recent IRS provisions granted for the benefit of employees 591/2 or older, the following information is required" asking for info that could lead to identity theft. One of the look up sites placed the call as being from the OK or MO area.The address in DC seems to be a UPS Store. I'm not falling for it.

I got the same letter as RDB above. It's a scam. I sure hope that no one falls for this ID theft attempt.   There is no singular, legal "R-9" form.  The holder of a persons 401K already has this information, as would the IRS.  These crooks just never quit. May God help them.  I'd hate to be in their shoes.
The address on the letter is: Employee Services 1380 Monroe St NW - Suite 641- Washington DC    Phone number:8878-598-0356   But! The business reply envelope they send in the letter, is addressed to: Employee Services - P.O. Box 140290 - Nashville, TN.   Seriously?!
Just google that address, and you can clearly see that it is a scam.

Correction:  The phone number is, 1-888-598-0356  Entered a "7" by type-o

I just got the same letter in the mail.  You are correct, the addresses are totally different.
Thank you and RDB for the additional information about this being a scam.  I thought it looked a little fishy.
What will they think of next!!!

Take the letter to the post office and have the postal inspectors look at it.  This looks like mail fraud to me.
