My Wells Fargo Visa debit card was hacked on 3 separate occasions in 2013 My card was charged $12.95 each time by a company called ICNetCo LLC in California. one at the end of August 2013, one in the beginning of October 2013 and the third in the second half of October 2013. This card had never been used prior to the attacks so it seems the number was stolen from either the Wells Fargo or Visa databases directly.

The first two charges used a phone number beginning with the 415 area code.  The third charge included this 888 toll free number.
