They Called to set up '' in-home '' health assessment. She knew my address. Told her to not call again and hung up. Call I.D. showed as 'Healthcare" with a graphic of a checkmark in a circle that literally read 'Legit' . Googled the number and it came up as a scam.

Got a call from 718-578-4640 asking me to call the number 877-391-3989.. What  up ?

So, what were the details of this "scam"?
"scam" tells so little.  Were they offering a fake loan?  Perhaps, taking you to court for a debt?  Was it maybe a "relative" in jail in Mexico and you need to send them bail money?  Or was it something different?
There are so many possibilities, yet all you could tell us was it was a "scam".

Received a voice message telling me she was calling me on behalf of Wellcare and to call (877)391-3989.

Ever since I joined Wellcare, I have received hundreds of these call. They say "We represent, partnered with, associated with, working with, or recommended by Wellcare". I call Wellcare, gave them the numbers and info and they say its not them or anyone they work with. I asked why do these scammers know my name, phone number and have Wellcare insurance? It started very soon after signing up with for Wellcare. Someone is selling scammers my info!!!

That might be a possibility.  It is also possible that WellCare or one of the companies that partner with them has had a data breach.  Or, it could just be that this is a company partnered with WellCare and the customer service person you talked to does not have a list of all the numbers that their partner companies use.
