a w-9 is an purposeless for the IRS, the sole purpose of a W9 form is to record the Social security number of the person filling it out.

It appears to be a scam oriented to taking money from doctors:

I got a call from the same #.  Very strange indeed.
Do you have the info so i can file a complaint with Federal Trade Commission and FCC?

This is a physician's office and I received a fax from the above number today. It is titled "Important Claims Processing Information. NPI Required". After requesting personal info about the doctor,  there's a sentence that reads "thank you for helping SAVE THE CHILDREN of Africa". Does anybody know yet who or what this is for?

Received a call to my business number from 619 677-2480 trying to sound like the IRS and requesting a response by pressing a number.The number cannot be called for confirmation.Total scam don't answer.

This number constantly faxes us tax forms.

It is SPAM!

Three Rivers Providers - We get a fax from this number 1 time each month requesting a W-9 form be filled out.

I am trying to find a phone number for this company and in doing so I found this page on google. I actually had a fax number listed as 619-230-1500 in my files so I faxed it and asked for a call back to confirm the fax. A person names Erica Fuerte calle dback and left a message but didn't leave a phone number, I was at lunch and have since been unable to contact her or get her to call back by sending faxes.

his was before I saw this page.

I hope this info helps someone out. If anyone finds a phone number for this "company" please let us know via this board.

I got a call on my personal cell phone, funny thing is, only a very few, my daughters and some close friends, have this number,  I don't give it out.  I have had this number for several years but I do  get some wrong numbers asking for a Docter's office, wonder if there is a connection

Received at work - an agreement page and blank W-9 from Three Rivers Provider Network.  Says at the bottom Save the Children of Africa.

Found this on Craiglist while looking for employment, glad I didn't call them.  619-230-0533 Jarred Pierce is the contact name.  Hope this helps.
