- SanjThis is a cam unfortunately I got into and shared all of my personal information including family member names. I was looking for a healthcare insurance and filled out an online form, a local number called me and said that they found a good healthcare policy for me ( no-deductible , no-out of pocket but with a co-insurance of 10%-15% for medical claims. They "appeared" to be like exactly a genuine company.
They will not provide you a policy document regardless I asked several time. Finally i signed up , after searching online the costumer service number they gave me ( 954-606-9070) i realized that it was a scam. I immediately called them, 4 people answered and transferred to each other and finally long waiting list with no answer. I hung up after 15- 20 min. Canceled my credit card immediately however they still have my SSN , DOB and address unfortunately- Caller: Simple Health
- MS ScammedOn October 10, 2017, I was inquiring about health insurance for my college aged child. A friend sent me a link to “Official-Plans.com”. I clicked on the link and answered a few questions and immediately received a phone call from “Mario” with “Simple Health” and he said he would help me find an insurance plan appropriate and affordable for my child. He proceeded to tell me he found a PPO nationwide plan which requires no referrals, choose your own doctor, no pre-existing exclusions, coverage all over the United States, doctor visits, diagnostic testing, medical, surgical and hospital coverage and no waiting periods. Out of pocket expenses would be doctor visits 20-30%, specialist 20-40%, medications $4-$12 for generics and $5-$20 name brands. There is even an RX App where you could enter your zip code, name of medication and milligrams which will help you locate a cheaper alternative. There is no deductible and there are out of network benefits as well. The following are NOT covered; Maternity Care, services in a mental health facility or substance abuse. Now for the big question, how much is this going to cost me? The monthly premium for this plan is $172.71. There is also a $125.00 enrollment fee that will be charged with the first payment for a total FIRST payment of $297.71. For each month after that the monthly premium will be $172.71 and there will never be a rate increase on this coverage. It will be a month to month plan and can be cancelled at any time with no cancellation fees. Additionally, this plan also includes a $110K Accidental Death/Dismemberment plan included in the monthly premium. Mario even searched my child’s doctor and said he is covered under their plan. Also, we may cancel this plan within 30 days. Sounds great…right? So we proceeded with the enrollment process which included giving my child’s name, DOB, zip code, SSN (for age verification), my name, address, phone number, SSN and email address. I opted for the coverage to begin in January 2018 due to the current coverage is through 12/31/2017. Mario said he needed a credit card to secure this special rate for January. So, I gave him my CC information. He told me the payment will come out of January 2, 2018 and coverage will begin oat midnight on January 3, 2018. The next step was to speak with another person who would be going over the policy information on a recorded line (for my protection) where I would need to answer a few questions. So, we did that. During the recorded conversation, the representative was giving me the same details that Mario had given me, and I confirmed each item he mentioned. Then he mentioned something about NCE Membership, which I made note of because I was going to ask a question about that. I was also given the following number “MCELM02017”, which I am not sure exactly what it was for, but I made note of it. After I finished with the recorded portion of the call, I was transferred back to Mario who said everything was set and the application will be processed for approval and I would receive an email confirmation once processing was completed. Mario gave me his direct line to call him if I had any questions or concerns. Mario’s number is 954-606-9070. I thanked him for his assistance and ended the call.
The next day I received a phone call from 954-398-1923. It was a female that asked for my child. I said I was the dad and they were not available, but could I help her? She asked if I handle health insurance for my child and I said yes and explained that I had just signed her up for a plan to start January 2018. The woman’s voice tone completely changed and said ok, thank you. I inquired as to who she was and why was she asking about my child. In a rude condescending tone she said she was with the Insurance Marketplace and was calling to offer health insurance to my child, but I had already signed her up for insurance so she could not help me (very short and curt). And I said ok and hung up. When I got home I googled the phone number and found that it was a scam number. It didn’t dawn on me that she was connected with the same organization I had spoken with the day before.
Fast forward to the morning of 10/13/2017. I realized that I had not received a confirmation email about my child’s health insurance coverage yet. So, I called Mario at 954-606-9070. The answering recording stated “Thank you for calling customer service”…no company name…interesting. I followed the prompts and ended up speaking with Mario. His voice tone and demeanor had completely changed from the bubbly, friendly person I had spoken with a few days before. I told Mario that I called to check the status of the application and if it had been processed as I had not received the confirmation email he said I would receive. Mario said (in a “why are you bothering me” tone) that I would not receive a confirmation email until the first payment was processed on January 2, 2018 because the account would not be established until that time. That sounded odd to me and I asked him if her coverage is guaranteed to begin in January and that I could not wait until then to find out if my child were approved or not. Mario said he didn’t understand. I explained to him that I needed to know that the coverage was guaranteed so my child would have health insurance coverage as of January. Mario said yes, the coverage would begin in January once they had received the first payment and then I would receive the confirmation email and welcome packet in the mail. Additionally, I asked Mario for the name of the insurance company my child was enrolled for. His answer was “Unified Health” which is under the “Multiplan Network”. There is no policy number yet because the policy essentially would not be created until they received the first payment in January. I said ok, thanked him and ended the call.
Something did not seem right about this. Then I remembered the phone call I received from the “Insurance Marketplace” on 10/12/2017 from the rude female. I decided to “Google” “Unified Plan” and “Multiplan Network”. The results showed a listing of different websites to choose from. One of them was from “Ripoff Report | NCE Unified Health One Unified Health Scam”. The report stated “This plan represented as insurance but is really a Discount Plan Boca Raton, Florida. The article was about another individual shopping for insurance that sounded basically similar to my situation. Then I “Googled” Mario’s phone number 954-606-9070. I found a site:

Health insurance scammer !!! They sound nice, and the guy sells you a policy that sounds great, but at the end you'll find out that it doesn't ...

There were numerous complaints listed for this number stating the exact same scenario I had experienced. Ultimately confirming my suspicions that I had in fact been scammed. I remembered from previously calling Mario at 954-606-9070, the automated system said there was an option for cancelling the policy, so I called back. I selected the option to cancel the policy and spoke with Nathaniel, Senior Case Manager. I told Nathaniel the reason for my call and he said he would be glad to help me. He asked me for my child’s name, the phone number on the account, the address and email all of which I provided. Nathaniel asked me to hold while he accessed the account. When Nathaniel came back, he asked me again how he could help me. Again, I told him I wanted to cancel the policy for my child as I had found another plan that was more suitable for their current situation. Since the policy had not actually started yet, I just wanted to cancel it and make sure they would not be charging my credit card on January 2, 2018. Nathaniel said he understood my issue and began to describe all of the “benefits” available under the policy and asked why I wanted to cancel the plan. Again, I stated my reason and said the bottom line is that I want to CANCEL the policy. Nathaniel stated to me that he could help me with this, but for my protection, it would need to be a recorded conversation. I said that was fine. Then Nathaniel said “I know you are the person that is paying for the policy, however, my child was the policy holder and he would need to record them stating that they wanted to cancel the policy. I asked him why they needed to speak to my child today when they did not need to speak with my child during my initial call. He said it is their policy and he was just “doing his job”. At this point I was tired of being nice. I threatened to contact the Insurance Commissioner for the State of Florida, my local news channel and that I would report all of the fraud complaints I had found established against their company and phone numbers. I told him to cancel the policy and email me confirmation that the policy had been cancelled and my credit card would not be charged. He stated he could not do that, but he would have one of his “upper managers” contact me to see if they could help. I asked for the manager’s name and he said he didn’t know their name because it was a different department. I told him never mind I would simply hang up and then take care of this in my own way. I also told him DO NOT have anyone else contact me regarding this, I was done dealing with them. About 1.5 hours later I received a phone call from 754-802-3837. This person was named Christopher Fanfan from the Compliance Department of “Simple Health” and he was calling to assist me with my situation. He asked me again for the information on the application so he could access the right account. I gave him the information and he found the account. I stated to him that I want to cancel the policy and receive an email confirming the cancellation and that my credit card would not be charged on January 2, 2018. Christopher stated that he will place a request for the application to be cancelled and that all of the personal information will be deleted from their electronic records. I said that is fine, but could I please receive an email for confirmation purposes. He stated that they could not generate an email for a policy that was not activated as the emails are automatically generated from active accounts. I even asked him if HE could simply send me an email confirming our discussion. He stated he could not. I asked him, what happens if my credit card is charged in January, he stated that will not happen as they are deleting all of the information so it would not be possible for the charge to occur. However, he did give me his direct number to call him at 754-802-3837 if I had any other questions or concerns. I “Googled” 754-802-3837 and found it has an unknown registrant.
I know I made a lot of stupid mistakes at the beginning of this nightmare, but I, just like many others trusted that I had been given a reputable website and received a call from a reputable company only to find out it was all bogus and completely misrepresented as to what this/these companies are actually offering under the guise of Health Insurance”. I am sharing this information everywhere I can and am sending this complaint to the Insurance Commissioner of Florida as I believe this company “Official-Plans.com” aka “Insurance Marketplace” aka “Simple Health” aka “Unified Plan” aka “Multiplan Network” and these phone numbers 954-606-9070, 954-398-1923, 754-802-3837 are preying upon unsuspecting individuals via “bait and switch” to get them to sign up and pay for a product they are completely misrepresenting. - ShawnHad a “policy” with these people for a few months after determining that COBRA was far too costly. Now have coverage with a reputable health insurer through my new company. Called and cancelled my policy with Simple Health on 8 different occasions and also sent a letter advising I am cancelling. Luckily I changed my bank so they could no longer bill my debit card (I had a feeling they would ignore my numerous above-and-beyond efforts to cancel the policy). Sure enough, I received a call on 12/1 from a Simple Health rep advising that their attempts to bill me for December was declined and I should provide an alternative payment method, lest I risk losing this policy. Still, 12 days later, I am still receiving phone calls and emails from them. I generally ignore them but I occasionally answer, tell them my story, advise them that their constant calls and attempts to bill me for a policy I have repeatedly cancelled clearly proves that they are, at the very least, incompetent or, more likely, just outright scammers and thieves. I always End the call by requesting that they stop calling. And another call follows several days later. And on and on it goes...
